If you or the student you are recommending meets the criteria discussed in then please continue with completing the entire application. Also, please provide the following required documents:
Personal Information
- Copy of Student’s U.S. Birth Certificate OR copy of valid U.S. Passport
- Permanent resident card (if applicable, front and back)
- Copy of pages 1 and 2 of Parent(s) federal income tax form for the most current tax year (1040 or 1040A, not W2 Form)
- Benefits letter from Social Security and/or Social Services for your most recent year (if applicable)
Information from the School
- Current school report card and copy of current school schedule
- Copy of Test Scores: State NJSLA, PSAT scores, and/or SAT scores
- Official school transcripts
- Essay/personal statement
- Three (3) teacher recommendation letters – English/Language Arts, Mathematics, Science
- Guidance Counselor recommendation
